What is the Chronic Disease Treatment Programme?
Our Chronic Disease Management (CDM) programme is specifically designed to improve the health and well-being of individuals with certain chronic conditions. This HSE-supported initiative provides structured monitoring to help manage your condition more effectively, at no cost to you.
What are the Advantages?
- Regular, structured reviews of your condition with both your GP and practice nurse twice a year, giving you four free visits annually.

Who Can Benefit from the Chronic Disease Treatment Programme?
You are eligible for this programme, at no cost, if you:
- Have specific chronic conditions such as Type 2 diabetes, asthma, COPD, heart failure, heart attack, angina, stroke, cardiac arrest, irregular heartbeat, or hypertension.
- Hold a medical card or GP visit card.
- Are 18 years of age or older.
How Do I Enroll in the CDM Programme?
To join the programme, simply contact us by phone or email, and we’ll assist you with the registration process.
Initially, we’ll walk you through the details of the programme and help you sign up. Together, we’ll review your overall health, focusing on your chronic condition and any related concerns. You may also need to visit our clinic for blood tests and other assessments.
After the review, you’ll receive a written care plan tailored to your needs.
Your Participation Matters
By joining the CDM Programme, you consent to the collection of specific information during each review. Your GP will record your name, age, chronic disease diagnoses, medical history, and details of any symptoms or tests since your last visit.
This data is vital for improving our understanding of chronic diseases. It helps the health service enhance its ability to detect, treat, and prevent these conditions, ultimately leading to better care for all.
